FaMED vs. Kenntnisprüfung (KP): Differences, Pathways, and Timelines
When foreign doctors begin their licensing journey in Germany, the bureaucracy can feel overwhelming. Terms like FaMED, FSP, Defizitbescheid, Gleichwertigkeitsprüfung, and Kenntnisprüfung (KP) are constantly mentioned, often without clear context.
The most crucial distinction you must understand early is: FaMED is a language exam; Kenntnisprüfung is a medical knowledge exam.
Passing the language exam is the prerequisite gateway before you can either work under a temporary license (Berufserlaubnis) or proceed toward your full medical license (Approbation).

Pathway 1 vs. Pathway 2: Gutachten or Kenntnisprüfung?
After you pass your FaMED exam, the licensing authority (Approbationsbehörde) evaluates your foreign university transcripts against the German medical curriculum (Gleichwertigkeitsprüfung):
Pathway A: Document Equivalence (Gleichwertigkeit über Gutachten)
- An official expert (Gutachter) analyzes your medical school transcripts, syllabus hours, and clinical internships.
- If no substantial deficits (wesentliche Unterschiede) are found, you receive your Approbation without taking the Kenntnisprüfung.
- Common for doctors from the EU/EEA and certain recognized non-EU curricula.
Pathway B: The Direct Kenntnisprüfung
- If significant curriculum deficits are identified (or if transcript evaluation would take over a year), doctors opt directly for the Kenntnisprüfung (KP).
- While preparing for the KP, you can work as an assistant physician with a temporary license (Berufserlaubnis) for up to two years in most states.

Doctors who view the FaMED simply as a hurdle to clear with the minimum score often struggle later in the Kenntnisprüfung.
In the KP, the medical professors test your clinical acumen in German:
- Patient Presentation: You must present a real hospital patient to the examination committee using the exact same SBAR/SOAP structure learned in FaMED.
- Clinical Fluency: If you struggle to articulate pathophysiological reasoning smoothly in German, examiners may interpret linguistic hesitation as medical incompetence.
