FaMED Recognition 2026: Which German States Accept the FaMED Exam?

9 min read

One of the most frequent questions international medical graduates ask before investing in exam fees is: "Is the FaMED examination accepted in my target German state?"

Because Germany's medical licensing system (Approbationsbehörden) is organized at the state (Länder) level, requirements vary considerably. While the traditional Fachsprachprüfung (FSP) is administered by individual State Medical Chambers (Landesärztekammern), the FaMED exam was developed by the Ludwig-Maximilians-Universität München (LMU) as an objective, scientifically standardized C1 digital alternative.

In this guide, we review the current 2026 recognition status of FaMED across Germany and how you can ensure your certificate is accepted.

Why Choose FaMED Over the Chamber FSP?

If your intended state of licensure accepts FaMED (notably in Bavaria and surrounding jurisdictions), taking FaMED offers distinct advantages:

  1. Scientifically Standardized Grading: Traditional chamber exams are notorious for subjective grading swings based on which senior doctor conducts the interview. FaMED utilizes objective OSCE criteria and blinded video scoring.
  2. Predictable Schedules & Faster Dates: In many states, doctors wait 4 to 9 months simply to receive an FSP date from the Ärztekammer. FaMED testing center slots are booked online with predictable turnaround times.
  3. No Catch Questions: Chamber exams sometimes test obscure medical trivia or regional dialect idioms. FaMED strictly tests C1 communication competence, patient safety, and clinical structuring.

How to Get Written Verification from Your Authority

Before booking your €490 testing slot on famed-test.de, send a formal request email to your licensing caseworker (Approbationsbehörde). Here is a tested German email template:

Betreff: Antrag auf Approbation / Nachweis der medizinischen Fachsprache (C1)
Aktenzeichen: [Ihre Vorgangsnummer, falls vorhanden]

What If You Move States Later?

Once you obtain your official medical license (Approbation or Berufserlaubnis) in a state that accepted FaMED (such as Bavaria), that license is recognized across all 16 German federal states. You are not required to retake a language examination if you subsequently relocate to work in NRW, Hessen, or Baden-Württemberg.
