FaMED Patient Presentations: Use SBAR as a Practice Framework

•5 min read

The doctor–doctor station contains two patient presentations. Its published duration is 23 minutes including reading; the current practice test allows at most 11 minutes per situation. The cases need to be kept separate.

You communicate relevant supplied information in professional German and answer follow-up questions. This is a language task, not a requirement to recall a treatment algorithm from memory.

SBAR is an organising tool

SBAR can give a practice presentation a clear route. It is not the official mandatory answer template or a numerical scoring rubric.

PartUseful question
SituationWho is being presented, and why?
BackgroundWhich reported symptoms and history matter?
AssessmentWhich findings and interpretation are actually supplied?
RecommendationWhat course, plan or requested next step belongs to this task?

A supplied case can call for a different emphasis. Do not force information into an empty category.

A short fictional source-to-speech exercise

Practice notes: Frau Beispiel, 54; cough for five days; no temperature value supplied; medication list available in the task; examination report not supplied.

A faithful opening might be:

Ich stelle Frau Beispiel, eine 54-jährige Patientin, vor. Sie berichte über seit fünf Tagen bestehenden Husten. Eine gemessene Temperatur ist in den vorliegenden Angaben nicht dokumentiert.

This is deliberately a short language exercise. It does not support adding normal vital signs, prescribing medication or announcing a diagnosis.

Respond to a follow-up precisely

If asked about information that is present, answer briefly and accurately. If it is missing, say so:

Dazu liegen mir in der Aufgabeninformation keine Angaben vor.

Distinguish „bereits durchgeführt“, „geplant“ and „von mir vorgeschlagen“. Never turn a suggestion into an intervention supposedly already performed. Clinical knowledge is not separately assessed in FaMed, but your wording must not distort the information.

Practise two cases independently

Make separate notes for each practice case. After speaking, compare your recording with the supplied source: did you mix identities, times, findings or plans? Then repeat only the unclear section. Use the differential-diagnosis phrasing exercise to practise degrees of certainty.

The written station has a different task: anamnesis from its video in complete sentences. Do not carry this presentation's facts into it.

A two-case drill: find the mixed information

Use these original fictional notes only for this language exercise:

CaseIdentityReported complaintDuration
AFrau Beispiel, 54Hustenfünf Tage
BHerr Muster, 41Knieschmerzenzwei Tage

Neither sheet supplies a medication history or examination findings. Present case A, put its sheet aside, then open case B. If you say „Herr Muster berichte über seit fünf Tagen bestehenden Husten“, you have moved both the symptom and duration from the other case.

A source-faithful opening for B is:

Ich stelle Herrn Muster, einen 41-jährigen Patienten, vor. Er berichte über seit zwei Tagen bestehende Knieschmerzen.

If asked about his medication, the exercise does not support „Er nehme keine Medikamente“. Say that the supplied information does not state it. A missing medication history is different from an explicitly negative one.

After your second attempt, check three things: identity, symptom with duration, and the distinction between missing and stated information. Then compare the language. This is a self-check, not the official grading rubric or a clinical management exercise.
