FaMED Arztbrief Writing Guide: SOAP Format & Time Management Under 20 Mins

The written station (Station 3) of the FaMED exam is a common pitfall for candidates. In the digital examination center, you watch a video of a physician-patient encounter and are given exactly 20 minutes to write a concise, grammatically clean medical report (Epikrise / Arztbrief).

Candidates who write lengthy introductory essays frequently run out of time before reaching their therapeutic recommendations or diagnostic plan.

By structuring your documentation according to the SOAP method, you ensure that no vital clinical data is missed while finishing safely within the 20-minute countdown.


The Anatomy of the SOAP Note

SOAP divides clinical documentation into four distinct quadrants:

S — Subjective (Was der Patient schildert)
O — Objective  (Was der Arzt untersucht und misst)
A — Assessment (Diagnosen, Differenzialdiagnosen & Beurteilung)
P — Plan       (Diagnostik, Therapie & weiteres Vorgehen)

Let us examine what belongs in each section during your FaMED exam:

SOAP Schema Arztbrief Structure


1. Subjective (Subjektive Angaben / Anamnese)

Here you summarize the chief complaint (Leitsymptom) and the relevant history using precise medical verbs:

  • Aktuelle Anamnese:
    "Der 62-jährige Patient stellte sich mit seit drei Tagen bestehenden krampfartigen Schmerzen im linken Unterbauch vor. Die Schmerzen strahlen in die Leistenregion aus und werden von Obstipation sowie Fieber bis 38,4 °C begleitet. Übelkeit oder Erbrechen wurden verneint."
  • Vorerkrankungen & Medikation:
    "Bekannte arterielle Hypertonie (Amlodipin 5 mg 1-0-0). Keine Voroperationen am Abdomen."
  • Allergien & Noxen:
    "Keine bekannten Medikamentenallergien. Gelegentlicher Nikotinkonsum (5 Zigaretten/Tag)."

2. Objective (Objektive Befunde / Status praesens)

Only document verified physical exam findings and vital signs. Keep sentences concise:

  • Vitalzeichen:
    "Blutdruck: 135/85 mmHg, Herzfrequenz: 82/min rhythmisch, Temperatur: 38,3 °C axillär."
  • Körperliche Untersuchung:
    "Abdomen: Weich, lokalisierter Druckschmerz und Loslassschmerz im linken Unterbauch (Sigma-Divertikel-Zeichen). Keine Abwehrspannung. Darmgeräusche in allen vier Quadranten regelrecht vorhanden. Nierenlager beidseits indolent."
  • Labor & Schnelltests (sofern vorhanden):
    "Leukozyten: 13.800/µl, CRP: 68 mg/l. Urinstreifentest unauffällig."

3. Assessment (Beurteilung & Epikrise)

This is the cognitive core where you synthesize findings into a working diagnosis:

"In der Zusammenschau der klinischen Symptomatik mit lokalisiertem linksseitigem Unterbauchschmerz, Fieber und deutlicher Erhöhung der Entzündungsparameter besteht der dringende Verdacht auf eine akute Divertikulitis (Hinweis auf Sigmadivertikulitis).

Differenzialdiagnostisch kommen eine Kolitis anderer Genese (z. B. ischämisch oder infektiös), eine Urolithiasis links sowie ein kolorektales Karzinom in Betracht."


4. Plan (Procedere & Therapie)

Specify exact diagnostic next steps and immediate pharmacological interventions:

  • Diagnostik:
    1. Abdomensonographie zum Nachweis von Darmwandverdickung und perikolischem Fettgewebsödem.
    2. Bei unklarem Ultraschallbefund: Abdomen-CT mit rektaler und intravenöser Kontrastmittelgabe.
    3. Koloskopie im entzündungsfreien Intervall (in ca. 4–6 Wochen) zum Malignomausschluss.
  • Therapie:
    1. Nahrungskarenz und Flüssigkeitssubstitution intravenös.
    2. Analgetische Therapie (z. B. Metamizol 1 g i.v. bei Bedarf).
    3. Bei unkomplizierter Divertikulitis mit Risikofaktoren: Beginn einer antibiotischen Therapie (z. B. Cefuroxim + Metronidazol).
    4. Stationäre Aufnahme zur Verlaufskontrolle.

5 Time-Saving Rules for the Digital Test

  1. Use Bullet Points in Objective & Plan: Examiners prefer clean lists over dense narrative blocks. It saves you 3–5 minutes of typing.
  2. Watch Your Medical Orthography: Misspelling standard Latin terms ("Appendisitis" instead of Appendizitis) will cost valuable linguistic points.
  3. Never Omit Vital Signs: Even if normal, always state: "Kreislauf stabil, afebril / subfebril".
  4. Draft the Assessment First: If you know your diagnosis early, formulating the Plan takes less than 3 minutes.
  5. Reserve 2 Minutes for Proofreading: Quickly check for subject-verb agreement and German compound nouns (Unterbauchschmerzen, Entzündungsparameter).
