How to Prepare for the FaMED Exam While Working Full-Time (6-Week Plan)

The overwhelming majority of international medical graduates preparing for the FaMED examination in Germany are not full-time students. You are likely working long shifts as a clinical observer (Hospitant), nursing assistant (Pflegehelfer), or managing family responsibilities in a new country.

When you arrive home exhausted after an 8-hour hospital shift, reading thick German internal medicine textbooks is unrealistic and ineffective.

To pass the FaMED exam while working full-time, you do not need 6 hours of daily study. You need 90 minutes of focused, high-yield simulation. Here is the 6-week blueprint designed specifically for working doctors.

The 90-Minute Daily "Triad" Formula

Divide your 90 minutes into three distinct 30-minute cognitive blocks:

Block 1: Audio & Vocabulary Intake (30 mins — Morning Commute / Lunch): Listen to clinical audio cases or review digital flashcards of medical German vs. lay terms using the FaMED App.

Objective: Master rapid neurological review of systems, time-of-onset questions, and Lumbar Puncture informed consent.

Week 4: Surgery, Orthopedics & Traumatology

Topics: Distal Radius Fracture, Femoral Neck Fracture, Cruciate Ligament Tear, Deep Vein Thrombosis.
Objective: Physical exam vocabulary (Status praesens), conservative vs. surgical treatment explanations, and radiation protection counseling.

Week 5: Mastering the Doctor-to-Doctor Handover (SBAR)

Focus: Synthesizing all previous cases into rapid, structured 5-minute doctor handovers.
Objective: Practice formulating differential diagnoses and defending diagnostic orders against senior doctor questions.

Week 6: Full OSCE Mock Simulations & Digital Interface Drills

Focus: Complete three full digital mock exams under realistic timed conditions.
Objective: Perfect German QWERTZ typing speed and build stamina for the 3-station circuit.

How to Leverage Your Clinical Shift for Exam Prep

If you are already doing a Hospitation (clinical observation) in a German hospital, turn your daily workday into active exam preparation:

Listen to Senior Physician Handovers (Frühbesprechung): Pay attention to the exact sentence structures senior consultants use to summarize overnight admissions.
Read Real German Discharge Letters (Arztbriefe): Study the hospital's electronic medical records to observe standard German phrasing for normal and pathological findings.
Practice Lay Explanations with Real Patients: When accompanying attending physicians, observe how they explain ultrasound findings, blood draws, and CT scans to patients without using medical jargon.
